I am a specialist knee physio through working at advanced clinical practice level in the NHS.  In order to get to this level you have to develop into an expert in all areas of the body before you can specialise.  I have worked as a musculoskeletal clinical specialist and first contact practitioner for a number of years, treating those with back, shoulder, neck, hip and ankle pain.
